Spiritual Reference Form

100% Changed™ applicants are required to submit two (2) spiritual life references in order to be considered as a candidate for the program.  What is a spiritual life reference?  It is a person, specifically a pastor, spiritual leader, or minister at your church, who can adequately provide insight into the participant’s spiritual life and ministry connection.

We ask that the applicant select individuals who are able to speak openly and honestly about their spiritual growth, character, and personal lifestyle, as this is the only way that we will be able to accurately determine how to best serve the applicants needs.
